Unlike in The Sims, Sims will get pregnant when they want to have a baby instead of a baby magically appearing in front of them in The Sims 2. Both male and female Sims can get pregnant, Sim pregnancy will only last for three days.

[edit] Day one
On the first day of pregnancy, the one after 'the deed', the Sim in question will begin to rush to the toilet and throw up, usually with a thought bubble of a dummy/pacifier signifying a baby. These spontaneous toilet prayers will cause a severe drop in comfort, hygiene and, understandably, fun.
You can tell if a Sim is pregnant before they begin to spew after the action 'try for baby' is complete. Unless certain mods have been installed, a lullaby will play.

On the second day the sim in question will wave the camera towards themselves, they will then change into maternity and the bump will appear. If the sim has a job, a message will appear informing the player that the sim is on maternity leave. For relationship points other members of the family can interact with it. Needs will drop for the sim a lot faster than usual, it may be advisable to let the spouses do the cooking. Interactions are more likely to go south.

Again the player will be directed to the attentions of the sim in question as the bump increases in size. As well as the drop in mood and social grumpiness, the sim will be unable to drive (although they can get to community lots other ways, they will not give birth there), use the swing or hot-tub.

At the end of day three the player will once more be directed to the pregnant sim. After waving you over, the sim will go into labor. This will autonomously call all other sims on the lot forward to watch the birth. After a short display a message will appear as the new sims genetics are being made up. What happens next depends on the power of your computer and the settings you have enabled. With the 'special camera' turned off, all that happens is the sim will spin on the spot and the baby will appear. With the camera turned on, a cinematic will play. There is a different one for normal and alien pregnancy. The spouse (usually) will come and see the baby when this is done.

For a female sim to get pregnant, she must try for a baby with a male adult, young adult, or elder. Female elders, young adults, and teenagers can not get pregnant. If pregnancy occurs on vacation, the sim will only exhibit signs when they get home.

For a male sim to get pregnant, they must have high logic skill, and if they stargaze using the Farstar e3 Telescope for a long time aliens will abduct them and they will return home pregnant with an alien. That is the only way male sims can get pregnant. Male elders, young adults, and teenagers can not get pregnant.
[edit] Pet pregnancy
If you have The Sims 2 Pets expansion pack, pets can also try for a baby and give birth to puppies or kittens. Birds and womrats cannot have babies. You must have a male pet, a female pet, and a pet house on the lot. The two pets must have a high relationship with their owner and with each other. Like sims, pets will also be pregnant for only three days, then they can give birth to up to 4 puppies or kittens. Their needs do not change, they can still work and their morphology stays the same.

- Sims can have twins. If this occurs the cinematic will only play once.
- Number of offspring is decided in the pregnancy
- Genetics and gender of the baby is why loading takes such a long time while the baby is being born. If your not happy with the baby, by re-loading the game without saving, you can have another run at it.
- Before Open for Business twins were very rare. The makers of the game changed the probability settings.
- While cheats such as 'TwinsR2cute' will not work, with Open for Business maxis introduced a cheats that does- Forcetwins. For this cheat to work the pregnant sim must be selected and pregnant.
- Eating cheese cake will increase the chances of twins.
- It is possible for a sim to become pregnant with anyone using the Tombstone Of Life And Death.
